Mr Mamun Rahman, a fully qualified Consultant Ophthalmologist (GMC no. 6048722) carries out the online consultation, and issues the prescriptions. The treatment is dispensed and posted from a separate pharmacy.
1. You must read the important safety information about the treatment. 2. You provide our Ophthalmic Consultant with essential health information by completing an online medical questionnaire. 3. If you have any questions you can contact our Consultant, or you may wish to have a face to face consultation. 4. Our Consultant checks the questionnaire answers before prescriptions are issued. You may require a face to face consultation before a prescription can be issued. 5. When a prescription is issued a confirmation email is sent. 6. The treatment is dispensed and delivered by a separate pharmacy.
Our Consultant may for safety reasons decline to prescribe the treatment. When a prescription is declined an email notification is sent. We will not take payment if a prescription cannot be issued.

Informing your General Practioner
You should inform your General Practioner about starting prescription treatments for eyelash improvement. If you would like our Consultants to contact your GP then please tick the appropriate box in the medical questionnaire.
